HealthPathways Project Officer
Salary: $68,250 - $78,602 per annum
LOCATION - Erina
HNECC PHN is seeking a part time (0.6FTE) HealthPathways Officer to join our team, based in Erina. HealthPathways is an online health information tool that supports primary care at the point of care. Working as part of the Central Coast HealthPathways team, the HealthPathways Officer will be responsible for ensuring that the operational aspects of the program are maintained within the resource allocation, and that content remains relevant and useful for health providers and collaborative stakeholders.
The successful candidate will have relevant tertiary qualifications in a health-related field and/or extensive experience in a primary health care environment to support this clinician led program.
The ideal applicant will have demonstrated skills and experience in developing and maintaining relationships and liaising with a range of organisations and health professionals (e.g. GPs, hospital clinicians and Allied Health professionals) and display experience in gathering information from a variety of sources and collating it in a concise meaningful manner. They will have excellent written and verbal communication skills. They will also have highly developed organisational and time management skills and a proven ability to undertake complex tasks in short timeframes, working autonomously where required to deliver high quality outcomes.
